Job Description: 

To act as a ‘single point of referral’ into community mental health support from the Route One to Wellness partnership from health professionals in Primary, Secondary, and Local Authority care

To assess and triage these referrals into services and activities provided by Orb Community Arts, the wider Route One to Wellness partnership, statutory and VCS organisations working within the developing HaRD Mental Health Hub & Spoke network, and other external agencies

Key Terms and Conditions of Employment

Post – Community Mental Health Keyworker

Hours – 30 hours per week

Salary Scale – NJC Pay scale 5 spine point 15 equivalent

Salary Range - £29,093 per annum FTE (based on 37.5 hrs full-time)

Location – Orb Community Enterprise, The Garage, Elephant and Castle Yard, Off High Street, Knaresborough. HG5 0EQ. The role will also require working at other Route One to Wellness, HaRD Mental Health Hub, and Better Connect partner premises and some travel to support onward participant referral or engagement with external agencies.

Accountable to – Director

Working with – The Orb team, the Route One to Wellness Project Co-ordinator, and Route One to Wellness partner Keyworkers when appropriate

Contract Start Date - to be agreed

Contract End Date - subject to ongoing funding

Holiday Entitlement – 28 days per year plus 8 bank holidays pro rata

Pension – automatic enrolment pension scheme provided in line with statutory regulation by National Employment Savings Trust (NEST): 5% employer’s contribution and 3% employee’s contributions rising in line with statutory requirements.

Probationary Period – the post is subject to a three-month probationary period.

Employee and Employer Notice of Termination -

Under two years’ continuous employment (but over 1 month): one week.

After two year’s continuous employment: two weeks.

For each additional full year of employment: 1 additional week up to a maximum of twelve.

Background to Orb

Orb Community Enterprise (operating as Orb Community Arts) is a leading Creative Health organisation, with over 20 years’ experience supporting people experiencing or at risk of poor mental health. It is based in Knaresborough but works across the wider Harrogate and Rural District (HaRD). Its aim is to promote positive mental health and provide better life chances for these individuals by using creative engagement as a driver for social inclusion, personal empowerment, increased skills and confidence and increased opportunities to move into volunteering, work or education.

Background to Route One to Wellness, the HaRD Mental Health Hubs, and the Better Connect Partnerships

Orb is part of Route One to Wellness, a VCS Mental Health partnership in the HaRD area that also includes Claro Enterprises, Mind in Harrogate District, and Wellspring Therapy & Training. This partnership offers people referred from Primary, Secondary Care, and Local Authority care a ‘shared menu’ of community mental health services via a single point of access, allowing for easier and quicker access to a holistic package of support.

The Route One to Wellness partnership is also working with Tees Esk & Wear Valleys NHS Mental Health Trust (TEWV), and North Yorkshire Council (NYC), to develop a Mental Health Hub and Spoke model. This will be delivered in collaboration with local ‘Community Anchor Organisations’ in Boroughbridge, Knaresborough, Pateley Bridge, Masham, and Ripon, to ensure that community mental health services have improved rural reach.

Orb also works closely with the Better Connect Skills partnership, a collaboration with a range of local skills and health providers, providing increased opportunities for progression focused personal development to those furthest from the job market.

Purpose of the Post

To assess and triage these referrals into services and activities provided by Orb Community Arts, the wider Route One to Wellness partnership, statutory and VCS organisations working within the developing HaRD Mental Health Hub & Spoke network, and other external agencies

To provide 1:1 Keyworker support to a caseload of individuals at Orb Community Arts to engage with services, develop skills, confidence, and resilience, and progress into further learning, volunteering or employment opportunities

Main Duties and Responsibilities

Referrals, Assessment, and Triaging (Route One to Wellness)

Acting as a single point of contact for referrals into Route One to Wellness and HaRD Mental Health Hub & Spoke services from Primary Care (including GPs, Social Prescribers, and First Contact Mental Health Practitioners), Secondary Care (including the TEWV Harrogate and Ripon Integrate Care Teams and Mental Health Services for Older People , the Local Authority ( including NYC Health & Adult Services, and the Living Well team)

Liaising with referral partners to support knowledge of the Route One to Wellness and HaRD Mental Health Hub & Spoke offer and how to access it

Providing a single Route One to Wellness/ HaRD Mental Health Hub wellbeing assessment that can be shared with Route One and Hub delivery partners

Triaging individuals to appropriate services within Orb Community Arts, the wider Route One to Wellness partnership (including Claro Enterprises, Mind in Harrogate District, and Wellspring Therapy & Training) organisations working within the developing HaRD Mental Health Hub & Spoke network (including Community Anchor Organisations, TEWV and NYC), and other external agencies when appropriate

Conducting short post engagement reviews with people supported through Route One to Wellness and the Mental Health Hubs, to provide understanding of impact and provide feedback to the original referring organisation

Liaising with external agencies to ensure we have all the required information to be able to support an individual into services.

Engagement and Progression (Orb)

Supporting a caseload of individuals to access services and interventions provided by Orb Community Arts

Supporting these individuals to access other services and opportunities with Route One to Wellness, HaRD Mental Health Hub, and Better Connect partners, and other agencies when appropriate

Providing these individuals with regular 1:1 meetings to support engagement, wellbeing, and progression

Offering short-term low level wellbeing interventions that promote self-care, independence, and wider life skills

Supporting individuals to set and achieve wellbeing and progression goals (including accessing volunteering, education, and employment opportunities)

Providing individuals with information, advice, guidance

Supporting individuals to overcome barriers to progression such as housing issues, or accessing appropriate benefits

Supporting individuals to undertake job preparation, work experience, work placements, volunteering, and education

Networks

Supporting the development of peer-to-peer support opportunities for those engaging with Orb support

Supporting people engaging with Orb, Route One to Wellness, and the HaRD Mental Health Hub support to influence services through participation in local service user forums and/or at system meetings organised by deliverers of mental health services

Liaising with colleagues within the Route One partnership and those within the Mental Health Hub network from Primary, Secondary, Local Authority, and VCS partners in Multi-Disciplinary Team Meetings to ensure individuals receive linked up and holistic support

Networking with and keeping abreast of local services and other sources of help relevant to individuals accessing support

Administration

Collating and maintaining accurate records of individual engagement with Route One to Wellness and HaRD Mental Health Hub services

Collating and maintaining accurate records of individual engagement with Orb services, including the requirements of funding partners such as the Better Connect partnership

Sharing appropriate personal data with Route One to Wellness, HaRD Mental Health Hub, and Better Connect partners and referring organisation to ensure linked up holistic support

Sharing anonymized data with the Route One to Wellness Project Coordinator and Better Connect to support project management and evidence impact

Other Duties

Participating in supervisions, performance management, staff development and training

Working towards individual and team targets.

Any other duties deemed reasonable in delivering the work of Orb Community Enterprise, and its partners at Route One to Wellness, the HaRD Mental Health Hubs, and Better Connect.
