Part-time Dementia Support Advisor – Harrogate and Ripon – 14 hours per week - £13.79 - £15.10 per hour

Dementia Forward is seeking a pro-active, motivated person, with a good understanding of the needs of people with dementia. The main focus of the job will be to provide a service for people with Dementia by providing information, advice, practical and emotional support from the point of diagnosis throughout the course of the disease.

The Dementia Support Advisors do this by providing home visits to clients covering a wide geographic area, to assess support needs and to prepare individual plans of care to meet client and carer requirements.

The Dementia Support Advisor will also coordinate and facilitate weekly wellbeing groups. The right person will be a good communicator and team player, who is well-organised and willing to learn new processes as part of the role.
