A range of support delivered in and around the home to enable people to live independently and manage age related long term conditions; including medication prompting, a full range of domestic tasks, preparation of meals and general help with daily living, escorted transport to GP, hospital appointments, memory clinics etc. Advice regarding care options is offered as well as a full health and social care assessment leading either to a tailored support at home package or referral to other agency.
