Home From Hospital in North Yorkshire

Home from Hospital is still taking referrals from professionals in the hospitals, community hospitals, and social care during the coronavirus pandemic. Residents can also refer themselves. The team will contact clients by phone to discuss any concerns and needs, such as shopping. They will try to ensure that the people they work with are safe and have their immediate needs met, and they will refer them to statutory and local voluntary sector services if required.

The Home from Hospital Service helps people aged 18 or over in North Yorkshire to be more confident and comfortable when returning home.
The team ease the process of settling back home - allaying anxieties and helping to resolve problems that may arise. The free service eases a patient’s return home, helps them to rebuild confidence and independence, and aims to prevent hospital readmission.

Support can be provided for up to 6 weeks and visits are usually weekly and for up to 1 hour. Over this period we can assist in re-building confidence and independence.
