Saint Michael’s Frailty Service
The project aims to support people in the Harrogate and Rural district who score four to six on the Rockwood scale for frailty and for whom frailty is starting to have a negative effect on daily life.
The support will be personalised to help people to have a better quality of life and feel less isolated or lonely – the project focus is on improving wellbeing, rather than clinical intervention.
Support will begin with assessing the needs, identifying goals and creating a plan to meet for up to 12 weeks, either at home, over the phone, or at Crimple House.
Voluntary organisations can request a referral form by emailing hospice-services@saintmichaelshospice.org. Anyone is also able to self-refer by calling 01423 200110.
