Dementia Forward have 4 vacancies for Dementia Care Coordinators 

This is an exciting opportunity to work within Primary and Secondary care supporting a coordinated approach to ensure people receive the right level of support throughout their patient journey with dementia.

Each post is for 35 hours per week.  Posts are based in:
Harrogate
Northallerton
Scarborough
York

Starting salary - £21,275 per annum 
5 weeks annual leave plus Bank Holidays

Candidates will need good I.T. and strong interpersonal skills.  Knowledge of dementia is a bonus.
